HealthLinc, a Federally Qualified Health Center headquartered in Valparaiso, IN with locations across Northern Indiana, has installed Forward Health Group’s PopulationManager® and The Guideline Advantage™. The Guideline Advantage is a quality improvement program of the American Cancer Society, American Diabetes Association and American Heart Association. The Guideline Advantage works to help improve patient health through prevention, early detection and management of cancer, diabetes, and heart disease.
The implementation of The Guideline Advantage was made possible by a grant from The GE Foundation and in collaboration with the Indiana State Department of Health, and leverages population health management tools from Wisconsin-based Forward Health Group, Inc.
“HealthLinc is dedicated to improving the health of our patients and the communities we serve. The Guideline Advantage and PopulationManager have given us the data we need to systematically drive population health improvement and disease management,” said HealthLinc CEO Beth Wrobel.
On September 15, HealthLinc East Chicago and HealthLinc Mishawaka will host media events with representatives from The American Heart Association, HealthLinc, and Forward Health Group all present to answer questions regarding the benefit of this program to the patients.
In 2014, all HealthLinc clinics were recognized as Level 3 Patient-Centered Medical Home by the National Committee on Quality Assurance. In addition to primary and preventive medical and behavioral health services, HealthLinc provides health and wellness education, dental services, optometry, OB services, chronic disease management, women’s and men’s health services, wellness checks and immunizations.
HealthLinc, Inc. is a not-for-profit Federally Qualified Health Center that provides primary medical, dental, and behavioral health services to people of all ages regardless of a patient’s ability to pay. HealthLinc clinics are located in Valparaiso, Valparaiso Porter-Starke Services, Michigan City, Knox, Mishawaka, and East Chicago, and accept Medicare, Medicaid, Healthy Indiana Plan (HIP), some commercial insurance plans, and uninsured patients on a sliding-fee scale based on their income.
